NEW palliative care pharmacy pack supports pharmacists, patients and families
Community pharmacists play an important role in supporting families and carers who are providing care for a home-based person with a life-limiting illness.
The Community Palliative Care Resources Pharmacy Pack is a new, free resource for pharmacists, developed by the caring@home project, that supports this care at home.
The Pharmacy Pack contains:
- hardcopy resources for pharmacists to use to support families and carers who are providing practical care, physical symptom management, and subcutaneous medicine management for a person at home.
- shelf talkers, to support timely ordering and stocking of medicines from the National Core Community Palliative Care Medicines List.
